Behavioral Health Case Manager - 32hr Work Week

We serve more than 60 families each year across 9 counties and 2 program sites in Idaho’s Treasure Valley. With integrated services including case management and behavioral health, CATCH's person-centered model of care offers the families that we serve individualized care, rental assistance, home visits, and stabilization supports.
The Behavioral Health Case manager reports to the Therapist/Clinical Director and works in a multi-disciplinary team of case managers, a counselor, and landlord liaisons. This position will provide behavioral health interventions and consultation to clients served to improve psychosocial functioning.

Responsibilities
- Ability to provide brief screenings and brief interventions to clients as well as complete diagnostic assessments.
- Is available for consultation purposes (as appropriate) and provides behavioral and cognitive interventions for clients.
- Identifies and/or receives referrals of at-risk clients and develops short-term goals to prevent psychological or physical deterioration.
- Respond to clients in crisis, providing appropriate support and resources
- Work within a team-based care model including participating in weekly team meetings, being available to consult with case managers, and jointly managing assigned patient populations with other case staff
- Determining the referral partnerships most needed to support CATCH clients, building a relationship with local behavioral health providers and maintaining functional pathways to care.
- Complete and maintain timely records and reports in an integrated electronic health record regarding client’s progress, services provided, and other clinical information.
- Complete treatment plans.
- Comply with all current CATCH policies, including HIPAA policies.
- Complete requested documentation that would assist client to access community resources (disability forms, client requested correspondence), or make appropriate referrals, as needed.
- Attend and participate in all staff meetings and other meetings as assigned.
- Portray professionalism to clients, other CATCH department personnel, referral sources, and the community during daily interactions and appearances.
- Adhere to company policies, practice guidelines, and professional code of ethics.
- Other duties, as assigned.
Minimum Qualifications
- Successful clearance of Department of Health and Welfare criminal background check.
- Master's degree in Social Work, Psychology or related field.
- Current professional license in the state of Idaho (LPC, LMSW, etc.).
- Proficiency in harm reduction, trauma-informed care, motivational interviewing, and psychosocial interventions.
- One year experience working in behavioral health provider conducting screenings and/or clinical care.
- Experience formulating a 5-Axis DSM IV diagnosis. Strong clinical written and verbal communication skills.
- Experience working with disenfranchised populations.
Preferred Qualifications
- Valid LMSW, LPC or equivalent in the State of Idaho.
- Bilingual in Spanish and English
- Two or more years' experience assessing clients.
- Experience working in a medical setting, community health setting, or as a member of a multidisciplinary team.
- Specialized trauma certification.
- Experience in working with populations experiencing homelessness
- Knowledge of EMDR.
- Knowledge of community resources.
- Knowledge of Medication Assistance Programs.

- Salary Range: $58,240 - $70,720, depending on experience
